Rotary Youth Exchange Application Process
 
  1. Click on the Forms link.
  2. Click on the appropriate application at the Forms page. Note: You need the free Adobe Acrobat Reader® software to read the application form, which is in PDF format.
  3. The application form is electronic, but you can not save the file. Therefore, the best approach is to first print a blank application form.
  4. Complete the application form off-line.
  5. Go back to the electronic form and complete the information on-line THEN PRINT THE COMPLETED FORM.
  6. Student and parents sign as indicated and pictures are added.
  7. Make 3 color copies of the completed application form. This yields 4 original application forms.
  8. Give all 4 original application forms to a Rotary club member.
  9. Rotary club members will interview student and parents.
  10. If student’s application is approved, the Rotary club representatives will sign all 4 original application forms and send them to the Rotary District office:

  11. The Rotary district office will interview student and parents by phone.
  12. If student’s application is approved, the Rotary district office will sign all 4 original application forms and send them overseas for matching with an exchange sister/brother.
