District Grant Forms |

District Grants are a tool for Rotary clubs and districts to support humanitarian projects that benefit the community. Districts can request up to 50 percent of District Designated Funds (DDF) for a grant to support multiple projects locally or internationally.

To apply for District Grants, clubs should submit the following forms: - District 6150 Future Vision Grant Application form (.pdf)
(complete online, save, print and submit)
- District 6150 Future Vision Grant Final Club Report form (.pdf)
Future grants cannot be made
until all funds spent for current grant projects are reported with
supporting documents and receipts.
