District Grant Forms

District Grants are a tool for Rotary clubs and districts to support humanitarian projects that benefit the community. Districts can request up to 50 percent of District Designated Funds (DDF) for a grant to support multiple projects locally or internationally.

To apply for District Grants, clubs should submit the following forms:

1. District 6150 Future Vision Grant Application form (.pdf)
   (complete online, save, print and submit)
   
   
   
2. District 6150 Future Vision Grant Final Club Report form (.pdf)
   Future grants cannot be made until all funds spent for current grant projects are reported with supporting documents and receipts.
