Commonly used Rotary Foundation Forms

Submit Rotary Foundation Funds using the TRF Transmittal Form (DaCdb)  Built into the District and Club Database (DaCdb) is an application and report generator that simplifies a club's reporting and submission of Rotary Foundation contributions.

Recognition / Contribution Form (123en_write.pdf) Use this form to submit a contribution to The Rotary Foundation, Transfer Recognition Points or to award a Paul Harris Fellow.

Multiple Donor Form (094en.pdf) Use this form when reporting several contributions to The Rotary Foundation (TRF)

Credit Card Contribution Form (trf_credit_card.pdf) Use this form to make a contribution to TRF with your credit card.

Recognition and Transfer Request (102en.pdf) Use this form when an individual, club or district is transferring TRF recognition points to an individual.

Publicly Traded Securitites Contribution Form (trf_public_securities_form.pdf) Use this form to contribute stock to TRF

Rotary Foundation Club Goal Report Form (.pdf) (2012-13 version) This writable PDF form must be completed and returned to the District Governor-Elect no later than May 1 prior to the President-elect assuming office on July 1. (Note in District 6150, President's-Elect turn this completed form it at PETS usually held in March.
